REGISTRATION FORM

You may fax or mail the form. Registrations are processed in the order they are received. Upon receipt of your signed application and tuition, confirmation of your registration will be mailed to you. All courses are approved for continuing education credit by the American Dental Association’s Continuing Education Recognition program (CERP) and Accepted by the Academy of General Dentistry. Cancellation Policy: OBI reserves the right to cancel or postpone any course dates due to unforeseen circumstances or insufficient enrollment. In the event of such cancel-lation, full tuition will be refunded or transferred to a future course date. There is a $300.00 cancellation fee for student cancellation within 30 days of the first session. Materials: A supply notebook will be provided with a list of equipment and materials necessary for the course.
